Airprox reports reviewed at the monthly meeting
Northolt Radar allowed the G650 pilot to fly into conflict
with the A380.
Contributory: 1. Timing and completeness of the Northolt
radar controllers’ handover.
2. The on-coming Northolt radar controller was distracted.
Recommendation: HQ Air Command review the tasking, manning
and supervision especially during Northolt extended hours operations.

A late sighting by the Chipmunk pilot and a possible non-sighting
by the EC145 pilot.
Contributory: 1. The high workload of the Coningsby LARS
controller resulted in him neither recognising the potential conflict nor
passing Traffic Information to the Chipmunk pilot.
2. The controllers and Supervisor did not assimilate that
the work-load was too high for bandboxed operations.
Oxford ATC did not integrate the DA42 and PA28.
Contributory: 1. The OJTI did not sufficiently mentor the
trainee or discuss the likely conflict scenario.
2. The Oxford MATS Part 2 provides inadequate guidance for integration
of the NDB/DME099 approach with visual circuit traffic.
Recommendation: Oxford reviews the
integration of traffic conducting instrument approaches and traffic in the
(HQ Air Trg)
The Farnborough controller released the CRJ2 into conflict
with the S76.
Contributory: 1. The Farnborough controller did not comply
with the VMC reduced separation criteria in the LoA.
2. Lack of effective internal and external coordination and
communication between the Farnborough and Odiham controllers regarding the
(HQ Air Trg)

Contributory: 1. Holding the DA42 at the VRP at low level
below a low cloud-base.
2. Communication issues between the Cardiff controller and
