The pilot of PA28(B) did not sequence with PA28(A).
Contributory: The PA28 pilots flew an extended downwind,
outside the bounds of the promulgated Kemble visual circuit.

2. The Hawk was not fitted with a TAS/ACAS.
Recommendation: HQ Air Command review coordination of
military activity in the Lincolnshire AIAA.
Plymouth Exercise Areas
A conflict in the visual circuit.
Contributory: The 2 pilots were on different R/T frequencies.
London City CTR
A procedural conflict in controlled airspace.
Contributory: 1. The Missed Approach Procedure and the SID were
not compatible in these circumstances.
2. The need for avoiding action was not immediately evident
to the ATCO.
3. The RJ85 pilot discontinued his approach rather than
continuing to the Missed Approach Point to go-around, which meant he caught
up with the RJ1H.
A non-sighting by the F15 formation members and effectively
a non-sighting by the S92 pilot.
Contributory: 1. The F15 formation flew through the TDA.
2. Their planning process did not give the F15 crews
information about the TDA.
Recommendation: HQ USAFE review their NOTAM notification
