Step 1 of 7: Instructions for use



Civil Pilots and Air Traffic Controllers should use the appropriate form to report or respond to an Airprox occurring within UK Airspace.

Pilot/Company identities are for contact purposes only and are not passed to any other organisations or kept on any databases.

If no other means are available, the UKAB will accept telephone reports.

Notes

Military ATCOs and pilots are to complete a DASOR in accordance with MAA RA1410, within 24 hours of the incident. Exceptionally, where no DASOR/ASIMS is available, a report can be emailed or faxed to the UK Airprox Board.

Airprox reporting procedures for civil pilots are promulgated in the UK AIP ENR 1.14 .

Submission instructions

The form will guide you through the completion process. When the form is complete and submitted it will be sent to the UK Airprox Board with a copy of the completed form also sent to the reporter’s email address, as supplied in the form.

If you have additional material that needs to be supplied please reply to the confirmation email and add the additional material as attached files.

Airprox outside UK airspace

Military pilots should contact the Flight Safety (or Ops) Branch of the appropriate Command/HQ. Privacy notice

The UK Airprox Board collects and stores your name and contact details in case we need to contact you as part of our investigation of Airprox incidents. We make sure your personal details are removed before passing Airprox data to third parties assisting in an investigation. Other than the final Airprox report itself, which will not contain your personal details, we will delete all data associated with the Airprox within a maximum of 3 years after the incident.

By consenting to this notice you are giving us permission to process your personal data specifically for the purpose identified. You may withdraw consent at any time, submit an enquiry or make a complaint by emailing FOI.requests@caa.co.uk.

Further rights as a data subject and contact details of the CAA’s Data Protection Officer can be found on the CAA website. You have the right to complain to the Information Commissioner’s Office (ICO) about the CAA’s processing of personal data.
